For Patients
Forms for new patients will be available within the patient portal after the appointment has been scheduled. These forms will be signed digitally, so there is no need to print forms on paper unless you would like copies for your records.
Insurance and Payment
Eucalyptus Health accepts the following plans:
- Aetna
- Anthem
- Blue Cross/Blue Shield
- Idaho Medicaid
- Medicare
- Tricare
We have applied with the following plans, and hope to accept in the near future:
- UnitedHealthCare
- Bright Path
Self Pay:
For clients paying out of pocket, initial appointments are $150 for 60 minutes, and follow up visits are $125 for 45 minutes. You will have ample time with the provider to explore your current concerns and medical and psychiatric history, as well as the plan for your care.
